We understand the task that presents itself once a patient has been recommended for home healthcare and your need to staff with a reliable trustworthy agency. Our 24 hour post discharge admission is always in effect for every patient. Care coordination for all disciplines needed in home is scheduled and we also follow up with the patient to make sure the services we are providing meets and exceed their expectations.
