SKILLED NURSING: These services are provided by Registered Nurses (RNs), Licensed Practical Nurses (LPNs) Includes health status assessments, teaching about diseases and treatment for clients and family members/caregivers, taking samples for lab tests, wound dressing changes, nutritional counseling, medication teaching and compliance, wound care, venipuncture, ostomy teaching and management, Infusion therapy and more.
HOME HEALTH AIDE SERVICES: These services are provided by certified nursing assistants who assist with Activities of daily Living, such as; assistance with bathing, medications set up, personal care, light meal preparation, and certain household task.
THERAPY SERVICES: Many therapies can be provided at home to speed recovery from illness, injury or surgery. These include:
Physical therapy to regain physical motion and strength, managing pain and enhancing safety. These are delivered by licensed physical therapist and Physical therapy assistants.
Occupational therapy to regain day to day skills such as dressing and feeding oneself, Helping people to regain skills needed to live independently, work with patients and or caregivers to enhance an individual’s ability to care for him or herself.
Speech and language therapy help patients to relearn speech skills that may be caused by brain injury or stroke, improve breathing or lung functioning, learning how to chew and swallow safely.
Medical Social Worker (MSW) They are Licensed Social Workers who assist families to coordinate community services; Help the patient and family to understand, accept and follow medical recommendations, and provide services planned to restore the patient to the optimum social and health adjustment within the patient’s capacity. MSW’s utilize all available resources, such as family and community agencies, to assist the patient to resume life in the community.
